What the rules require.

COMAH 2015 places two duties at the centre of personnel accountability. Regulation 5 requires a Major Accident Prevention Policy (MAPP) and, for upper-tier sites, a full safety report demonstrating that all measures necessary have been taken. Regulation 7 requires adequate emergency plans — internal and, via the local authority, external — that can be put into effect without delay. In practice, both hinge on being able to account for people quickly and accurately during a loss of containment.
